Provider groups participating in MN Community Measurement are listed below. Provider groups are listed first. If any clinics are affiliated with a group, a closed folder (
) appears before the group name. Click the gray arrow, an opened folder [
] will appear and a list of affiliated clinics will display as items (
) listed below the affiliated clinic. Click on any group or its affiliated clinic to view the provider group's profile. Clicking the gray arrow next to an opened folder [] will close the list of affiliated clinics for that provider group. Note that clinic pages will display the profile of their affiliated provider group.
